Pet Plans - Part II

1. Coverage: Accident and Illness. 
2. Limits: No annual limit.
3. Reimbursements: 50%-90% of covered vet bills; options vary based on pet age & breed.  
4. Deductibles: Annual deductible of $100 to $1,000. 
5. Enrollment: Clinical exam is required prior to enrollment. (Does not enroll pets older than 14 years old. No new enrollment for pets older than four in New York state).

Coverages:

• Hereditary & Congenital Conditions covered (Dogs 6 years or older are not eligible for hip dysplasia coverage). 
• Chronic conditions covered. 
• Dental issues not covered. 
• Diseases preventable by vaccines or preventive medication are not covered. 
• Sick visit exam fees not covered. 
• Diagnostics, including Radiology & Lab Tests covered. Surgery, 
• Emergency Services & Hospitalization covered. 
• Specialized Care covered.

2. Annual Limits: $5,000 to unlimited. 
3. Reimbursements: 70%, 80% or 90% of covered vet bills. 
4. Deductibles: $100, $250, $500. 
5. Eligibility: All Pets Ages 8 Wks. & Up Are Eligible to Enroll. 

Coverages: 

• Hereditary & Congenital Conditions covered. Chronic Conditions covered. 
• Preventable diseases covered if the illness occurs following vet’s vaccine recommendations.
• Dental Issues covered. 
• Sick Visit Exam Fees covered. 
• Diagnostics, including Radiology & Lab Tests covered.  
• Surgery, Emergency Services & Hospitalization covered.  
• Specialized Care covered.

3. Reimbursements: 65%, 70%, 80%, 90%, or 100% of covered vet bills. 
4. Deductibles: Annual Deductible with various pricing options. 
5. Eligibility: All Pets Ages 8 Wks. & Up Are Eligible to Enroll. 

Coverages: 

• Hereditary & Congenital Conditions covered. 
• Chronic Conditions covered. Preventable diseases covered if the illness occurs following vet’s vaccine recommendations.. 
• Parasite Infections not covered 
• Dental Issues covered. 
• Sick Visit Exam Fees covered. Diagnostics, including Radiology & Lab Tests covered.  
• Surgery, Emergency Services & Hospitalization covered.  
• Specialized Care covered.

5. Eligibility: All Pets Ages 8 Wks. & Up Are Eligible to Enroll.. However, there are health restrictions. 

Coverages: 

• Hereditary & Congenital Conditions covered. 
• Chronic Conditions covered. 
• Preventable diseases covered only if the illness occurs following vet’s vaccine recommendations and medication advice. 
• Dental Issues covered. (Dental exam and cleaning (if vet recommended) required for pets 3 years & up 13 months prior to a periodontal disease claim). 
• Sick Visit Exam Fees covered (optional for an additional monthly fee). 
• Diagnostics, including Radiology & Lab Tests covered.  
• Surgery, Emergency Services & Hospitalization covered.  
• Specialized Care covered.
